Breakers, a bar-restaurant in West Salem, Wis., reopened Thursday after an inspection by the La Crosse County Health Department.

The establishment had been closed since Monday because it did not have licenses to operate a tavern and a restaurant.
The bar-restaurant is now restricted to serving only pre-packaged food.

Inspectors found nine violations, including two for major risk factors, during the inspection. One major violation was for improper sanitizing techniques, according to the inspection report.
